As a result of the coronavirus pandemic, and as recommended by the Texas UIL Medical Advisory Committee (MAC) and team physician Dr. Andrew Parker, no student physicals will be conducted at Melissa ISD as traditionally scheduled in May.
All students participating in athletics, cheerleading, and/or band are required to have a physical on file. Below are the recommendations for student physicals for the 2020-2021 school year.
Students who currently have a valid 2019 UIL preparticipation physical examination (PPE) on file must still submit the medical history form for the 2020-2021 school year. The medical history form is the first page of the PPE and does not require the student to see a doctor for clearance, unless the student answers “yes” to any of the questions numbered 1-6 (numbered 1-21 on the online form).
The medical history form, along with all other required UIL forms, must be submitted online through RankOne in order for your child to be eligible for participation. Click here to access RankOne. Please download the RankOne Parent/Public app for assistance in determining the eligibility status of your student, and to access to other useful information.
Students who do not have a valid PPE on file are advised to see one of the following for clearance: pediatrician, physician, physician’s assistant, chiropractor, or nurse practitioner. It is not recommended that students go to the emergency room or urgent care for physicals during this time.
If you have any medical or RankOne questions, or have difficulty securing an appointment for your student’s physical due to the coronavirus pandemic, please contact Jose Mendez, Head Athletic Trainer, at firstname.lastname@example.org for assistance.
