Teen Screen Overview & Consent Form
Parents and guardians of students in 6th-12th grade will receive an overview of Teen Screen with information about a 10-minute questionnaire that will be offered at school on the dates below. The questionnaire will only be offered to students whose parent/guardian has submitted the consent form to the school.
