Request for Verification of Enrollment and Attendance (VOE) Form

Melissa High School students and parents/guardians in need of a Verification of Enrollment and Attendance (VOE) form must submit an online request, linked below, at least one (1) business day before the VOE form is needed.


VOE Eligibility & Guidelines

  1. The school considers the student currently enrolled at the time the student applies for the VOE, and
  2. the school awarded the student credit for each class the semester prior to the application for the VOE.

In order for a VOE to be issued, the student must be currently enrolled AND either: 1) met minimum attendance for class credit (90 Percent Rule) in each class they were enrolled in semester, 2) received credit for all courses taken in the previous semester, OR 3) has complied with the conditions established by the school to receive the VOE form.
