Request for Verification of Enrollment and Attendance (VOE) Form
Melissa High School students and parents/guardians in need of a Verification of Enrollment and Attendance (VOE) form must submit an online request, linked below.
A currently enrolled student seeking a driver’s license shall submit the Texas Department of Public Safety Verification of Enrollment and Attendance Form (VOE), signed by the parent, to the campus central office at least 10 days before it is needed. The district will issue a VOE only if the student meets class credit or attendance requirements.
VOE Eligibility & Guidelines
The school considers the student currently enrolled at the time the student applies for the VOE, and
the school awarded the student credit for each class the semester prior to the application for the VOE.
In order for a VOE to be issued, the student must be currently enrolled AND either: 1) met minimum attendance for class credit (90 Percent Rule) in each class they were enrolled in semester, 2) received credit for all courses taken in the previous semester, OR 3) has complied with the conditions established by the school to receive the VOE form.
