Required Information for UIL Participation

Melissa ISD students in grades 7-12 who are participating in UIL athletics, band, cheerleading, and colorguard are required to have certain information on file with the school each year in order to be eligible for participation in their desired program(s).

All required documentation must be completed annually in Rank One by the program deadline. Follow the instructions outlined below to access Rank One and complete your student's required information for the upcoming school year.

  • The student’s parent or guardian is required to print the pre-participation physical examination (PPE) form to complete with a physician, pediatrician, physician’s assistant, chiropractor, or nurse practitioner.

  • All other required documentation must be completed electronically in Rank One by the custodial parent/guardian and participating student. The parent/guardian and student are responsible for reading and completing the forms as they pertain to them.
