Melissa Middle School parents/guardians should have received an email with information regarding the Youth Check wellness check-up program. Youth Check is a nationally-recognized program developed to identify risk factors associated with depression, anxiety, and alcohol and substance abuse. The program is free and completely voluntary and confidential.

No later than Wednesday, Nov. 2, 2022, please read the Youth Check Parent Letter & FAQs (Carta para padres de jóvenes y preguntas frecuentes) and complete the Parent Consent Form(en inglés y español) to let the school know whether you want your teen to participate in the Youth Check screening at school on or around Friday, Nov. 11, 2022. If you are unable to complete the electronic consent form, please download the paper form in English or in Spanish to print and return to the school.
