All students participating in UIL athletics, cheerleading, and/or band are required to have a physical on file with Melissa ISD. Below are the recommendations for UIL student physicals for the 2021-2022 school year.


  • High school students participating in band and/or cheerleading can receive their physical on June 7, 2021 between 8:00 a.m. and 12:00 p.m. at the Melissa Arts Center, located at 3030 Milrany Ln. Physicals will be conducted for free in partnership with Questcare Medical Clinic.
  • High school students participating in all other sports can receive their physical on June 2, 2021 between 8:00 a.m. and 12:00 p.m. at the Melissa Championship Center, located at 3030 Milrany Ln. Physicals will be conducted for free by Texas Health Sports Medicine.
  • Middle school student-athletes should receive their physical exam from their family physician. It is the philosophy of our medical providers that students ages 12 to 14 should be evaluated by their family physician at least once a year, as there are immunizations that must be maintained for children in that age group.

As a reminder, your student’s medical history form, along with all other required UIL forms, must be submitted online through RankOne each year in order for your child to be eligible for participation in UIL athletics, cheerleading, and/or band.
