Melissa ISD students in grades 7-12 who will participate in UIL athletics, band, cheerleading, and colorguard during the 2022-2023 school year are required to have certain information on file with the school in order to be eligible for participation in their desired program(s).

All required documentation must be completed each year in Rank One according to the program deadline. Visit www.melissaisd.org/uil for instructions on how to access Rank One and to complete the required information for a participating student.

UIL requirements include an annual student physical. The student’s parent or guardian must print the pre-participation physical examination (PPE) form to complete with a physician, pediatrician, physician’s assistant, chiropractor, or nurse practitioner.

Clinics for student physicals will not be offered this year at Melissa ISD; however, a number of Melissa-area clinics are offering this service, as outlined below.

  • QuestCare: Free with in-network insurance, $25 for out-of-network.
  • CommunityMed: Free with in-network insurance; $49 for out-of-network cash pay. Register a student for a physical online at communitymedcare.com/physicals.
  • Texas Health Resources: Contact the clinic for information.
