As a reminder, Melissa ISD sent home two paper forms last week that need to be completed by parents and guardians and returned to the school by Wednesday, Oct. 6, 2021. These forms include:
- Student Directory Information Release - This form asks the parent/guardian to indicate their current wishes regarding the privacy of their child’s directory information.
- Internet Safety Policy & Acknowledgment Form - This form outlines the Melissa ISD Internet Safety Policy and asks the parent/guardian and student to acknowledge that they have reviewed the policy.
Students in Pre-K through 5th grade received the forms in their weekly folders. Students in 6th through 12th grade received the forms during their advisory period. If for some reason your family did not receive the forms, please click on the links above to download and print the forms to be completed and returned to the school.
